Cost and Insurance

We are in network with the following insurance plans:

If your insurance is not listed above we are not in network. We can provide a superbill/invoice for you to submit to insurance for out of network reimbursements.

Private pay rates are available upon request for those without insurance, those who do not want to use their in network plan, or those who have out of network insurance plans. Please email info@cmbh.health to inquire about private pay rates.

Forms & Paperwork

All forms, including those for disability, accommodations, ESA letters, and others, are completed during scheduled appointments.

No Show and Late Cancellations

All appointments must be cancelled within 2 business days prior to the scheduled visit. No shows and late cancellations will be charged $150.00 for the missed visit.
